Charlotte Health Care Center is currently accepting applications for a part time Discharge Planning Assistant with experience in long-term care and ! discharge planning. MFA provides a high volume of short term care services and treatment. We are seeking a candidate that can provide comprehensive medically related social services for a mixed caseload. Responsibilities include MDS, RAPS, Care Plans, Assessments and timely documentation operating within the framework for the company's standards for best practice in accordance with all state and federal regulations. This individual must also have the ability to help patients and families cope with the recovery process, financial planning, and discharge preparation.
