The Chief Nurse LTC/CLC provides executive leadership and assumes continuing and substantial responsibility for the coordination and evaluation of integrated programs that cross service and/or discipline lines and influence organizational mission, vision, values and strategic priorities. The Chief Nurse for Continuing Living Center (CN/CLC) assumes this responsibility for the 149 bed Community Living Center (CLC) which provides services for rehabilitation! , skilled care, respite, restorative care, long stay spinal co! rd injury, continuing care and hospice and palliative care. The CN/CLC is responsible for the program operations of the CLC. Within the program operation responsibilities include the lead of the CLC Clinical Guidelines and Policies, CLC Cultural Transformation Committee, CLC Interdisciplinary Staff Committee, co-lead of the CLC Admission Screening Committee, CLC and Performance Improvement Committee. Also within the program operation responsiblities is included ensuring that the clinical services offered by the program are in compliance with local and VHA Standards and Policies as well as accreditation standards for LTC.
He/She is responsible for the strategic planning and operational management of the CLC and the development and continued effective functioning of the interdisciplinary health care team. He/She collaborates with other CLC CNs' Veterans Integrated Service Networks (VISN) Leadership and VA Central Office staff on issues of program development. He/She ! is responsible for the interpretation and communication of VA policies/directives, The Joint Commission Standards and the Federal Regulation Tags to the LTC interdisciplinary team, Service Chiefs, Executive Leadership Team and others. Assists the CLC team in development and implementation of local CLC policies and procedures to include performance improvement, patient safety, utilization review, emergency preparedness, and safety issues. He/She is responsible for the effective operation of CLC Programs to include the Restorative Program, Hydration Program, Enhancing the Dining Experience Program and Walk to Dining Program among others.
He/She manages human resources for the CLC including: selecting qualified staff, orients and mentors the staff, and promotes staff development and professional growth to ensure competency. The CN CLC functions as Co-Chair of the CLC Admissions Committe, Co-Chair of the CLC Performance Improvement Committee, Chair of the Cultur! e Transformation Committee and the CLC Interdisciplinary Meetings. Is r! esponsible for completion of the Joint Commission Periodic Progress Review, the CLC Electronic Inventory List (EIL). Develops and implements the yearly budget for the Culture Transformation Fund Control Point (FCP) and functions as the approving official for this purchase card. Works closely with the Acute Spinal Cord Injury and Disease Service Chief to assure all candidates for Long Term Care Spinal Cord are referred to the CLC for services needed and maintains/actives the waiting list. The CN/CLC has vast expertise in Geriatrics and Long Term Care and serves as an expert for peers, populations, providers and other stakeholders in promoting and implementing all CLC policies, directives and programs. He/She supervises the CLC Nurse Managers, MDS Coordinator, Clinical Nurse Specialist and Program Support Assistant.
