The Adult/Family Nurse Practitioner will be asigned to the Community Living Center (CLC) program under Geriatrics and Extended Care Services. She/he will provides primary health care to selected patients with chronic, stable conditions or acute, episodic illness in the CLC, focusing on health promotion, disease prevention, health maintenance, illness, and disability minimization and Palliative care of the Terminally ill. Adult Nurse Practitioner use of the nursing process to provide primary health care (m! anaging minor, acute, and chronic conditions) to veterans in t! he Community Living Center, assisting nursing staff and other health care providers in identifying appropriate interventions for geriatric inpatients; organizing performance improvement activities related to geriatric patients; enhancing continuity of care for older veterans by arranging appropriate nursing care follow-up with community agencies and outpatient clinics; leading and participating in family meetings; providing educational opportunities for VA and CLC nurses; involvement in clinical research or application of research findings to clinical practice; consulting with VA and community providers to determine appropriate geriatric services; participating in program development; participating in care planning; and participating in interdisciplinary meeting at the Community Living Center Setting. The Adult/family Nurse Practitioner functions is an integral member of the health care team, working in collaboration with the nursing staff, physician, social workers, psycho! logists, and other health care providers to enhance the overall care of older patients at the Community Living Center, including Hospice care.
